This is a horizontal form that prints on the front of the 9×12 trip record envelope. Further customizations are possible upon request. To request changes, please make sure you check the “send me a proof” button before adding the trip envelope to the cart. Once we receive your order you’ll receive a digital proof. The digital proof will show you a ready-to-print template. Review the template and make sure the information fits your needs. When you review the envelope you’ll find the following information: Two lines for your drivers, Date, Truck and trailer number lines, Commodity, Shipper, Load number, Empty from and loaded from, Odometer start, finish and total. The first chart is for truck fuel which already has the following columns: date, city, state, gallons purchased, cash and price per gallon. The lines under it are for reefer fuel. The mileage box holds state, odometer reading, highways, and miles traveled. Driver’s signature and certification is at the bottom.
