This is a fully customizable template where our customers are able to request changes during the proofing process. Your company information prints at the top left hand side of the form. Your personal customization includes your company logo, name, street address, phone numbers and tag line. This work order form records the date of the job, customer order number, customer name, and address. Below the customer information is the machinery’s make, model, SN number and HMR details. The name of the mechanic working on the job goes last. Beneath this first set of lines is the description of labor. There are 12 lines to describe the work being done as well as columns for the hours, rate and total. The top right of the form is a table for recording the parts used, the quantity and amounts. There is even an extra column for any additional costs that occur. In addition to all of this, three lines are provided for any other expenses.
